Kindergarten - 5th Grade,  (every Wednesday September - May from 6:30pm-7:30pm)

As children enter into the pivotal elementary years, Awana ensures that kids are not only knowledgeable of God's Word but are able to apply His truth to their lives and face anything as disciples of Christ. Please complete this Awana registration form for the 2023-2024 Awana year.  We look forward to seeing your child here at AWANA!

I authorize any church representative to transport my child at his/her discretion in case of an emergency. I give my authority and consent to medical and surgical treatment as my be needed in the judgement of the treating physician and accept financial responsibility for such treatment. I have full knowledge and consent for my child to participate in all activities during 2023-2024 and to be transported by Northside Baptist Church and it's representatives. I release Northside Baptist Church and it's representatives from liability in case of accident or injury.

Therefore, I agree to indemnify and hold harmless the church, its trustees, pastor, associate pastors, deacons, its members and designees from any claims arising out of the use of my photograph(s). The church reserves the right to discontinue use of any photograph(s) without notice.
